Important Note for a New patient: Just a single session of Pelvic Floor Therapy Treatment is not valid for purchase by a new patient. This is just an add-on maintence session, offered only after at least One of the 6 session package has been utilized. So as a new patient please buy a new package and not a single session the firs time.
Cancellation Policy
What sets us apart from competitors offering similar services? We just don’t set up patients passively on the machines. We first do a thorough initial evaluation on your condition and set up a personalized treatment plan for you, with real measurable and objective goals and progress. Our “Treatment Packages” not just only include passive treatments but include a lot of value added services, other related treatentments and personalized consults which are fine-tuned with each session. Each session gives you access to an experienced medical professional as every session is supervised by a senior experienced staff member. We keep evaluating your progress on a regular basis in consultation with you to keep providing you with best personalized care. Please read it carefully. I understand and acknowledge that these services are NOT covered by most insurance plans. I have agreed to pay out of pocket for this volunteer treatment. I also understand that PPT has a "Late Show, Late Cancelation and a No Show" policy. Under this policy, I need to give PPT a minimum 24 hour’s prior notice for any change of appointment or cancellation. There will be no charge for any change, if I give at least a 24 hours prior notification to PPT. If I will be late by an hour or no show for my appointment, I will be charged for my missed visit for that day. At PPT, we accept prior notice by a phone call, text message or by an email.
